Please Note:Apply Monday-Thursday from 8:00-11:00 a.m. and 1:00-4:00 p.m. and Friday's 8:30-11:00 a.m. and 1:00-4:00 p.m.
Charge for this service is $10.00 with valid government issued photo identification.
The local Health Center can also provide addresses for requesting out-of-state Birth Certificates.
